Organs-on-chip allow for establishment of physiologically relevant models of human tissues and organs, and specifically of blood-brain barrier (BBB) and neurovascular unit (NVU) in their normal or pathological state. BBB/NVU integrity and functions are compromised in course of different diseases or injuries, like traumatic brain injury, stroke, multiple sclerosis, arteriovenous malformations, vascular dementia, Alzheimer?s disease, atherosclerosis, brain cancer, inflammation, genetic disorders, and many more. In this review we discuss main BBB/NVU properties, and latest achievements in healthy and pathological human BBB microfluidic models development. The growing evidence indicates that for human in vitro BBB models (and especially when recreating pathology) it is beneficial to use human induced pluripotent stem cells (iPSC). Besides the right choice of cells, it is obvious that static in vitro models are far from physiology, that?s why microfluidic human iPSC-based BBB models are of particular interest. Such models are potentially useful for development and testing of new neuropharmaceuticals, and development of strategies to prevent the breakdown of BBB/NVU in the course of certain disease.
